Registration Forms for High Holy Days 2022

Note on COVID-19: We are planning hybrid services for the High Holy Days: you may choose to attend in person or virtually via Zoom webinar. Some services and activities will not be streamed to Zoom - see schedule for details. Most in-person services will be held indoors and masks will be strongly recommended. Services will be held in the sanctuary; overflow seating with services streamed to a monitor will be available in the tent and/or other rooms in the building with fewer people. Meals will be held outdoors in the tent.
If you register to attend in person you will also receive information on attending virtually should the need arise. Anyone with COVID symptoms is asked to attend the service virtually.
If health conditions deteriorate, we will, of course, update you as to any changes we must make for the well-being of our Eitz Chayim family.

Members registering to attend virtually only: if you have renewed your membership, you will automatically receive an email with a link to join services virtually.

Additional Forms for High Holy Days 2022

Meal Information form: please fill out this form if you would like to attend the Rosh Hashanah lunches or the Yom Kippur break fast.

Child Care form: please fill out this form if you would like to sign up for child care during certain services.

Yizkor and Tzedakah form: please fill out this form if you wish to make additional contributions (flowers for the bima, extra contributions to Eitz Chayim) and/or submit names for the Yizkor memorial service.

Children's Services form (non-members): please fill out this form if you wish to attend only the morning Children's Services (1st day of Rosh Hashanah and Yom Kippur). Tickets will only be good for these services.
